Barriers and motives among advanced cancer patients regarding power of attorney and medical directives

Background: Advanced Care Planning allows patients to acknowledge their preferences regarding their medical care and to ensure their fulfilment.
In Israel, the issue was defined by law, however, a minority of patients have completed the forms (Power of Attorney/Advanced Directives). To the best of our knowledge, no research has been performed on Israeli patients' attitudes regarding this issue.
Objectives: To Identify the barriers and motives of patients and staff members regarding Advanced Care Planning.
Method: A survey research was carried out which included two phases: (1) patients completed a questionnaire evaluating their level of knowledge regarding the issue. Patients who were asked to complete the forms, met a staff member for a semi-structured interview. (2) Staff members completed a questionnaire evaluating their level of knowledge and perspectives regarding the issue. Qualitative and Quantative analysis was carried out.
Findings: Patients rated information and open communication with family and staff as the main enabling factors. Lack of a close relative who will agree to cope with the responsibility was the main barrier. Patients and staff rated their main motives as making sure the best medical decisions will be made and avoiding unnecessary medical procedures.
Conclusions: Aspects related to communication were found central to the process of ACP.
Recommendations: In order to promote the issue, guidelines and training programs, with practical and active workshops, should be established for medical staff. This will ensure a discussion compatible with patients' needs and will increase staff’ sense of efficacy during the process.
